Planning Ahead: Advance Care Planning for Chronic Illness

Planning Ahead

Advance Care Planning for Chronic Illness

Date: Saturday, November 14, 2026
Time: 12:00 PM – 2:00 PM EST

About This Workshop

Join Megan Stevenson, LCSW, CFSW and Destiny Davis, LPC, CRC for a conversation about something most of us avoid until we can't anymore: planning ahead for our own future healthcare. Advance care planning isn't just paperwork it's an ongoing process of understanding your values, naming the people you trust, and making sure your voice is heard even if you can't speak for yourself in the moment.

We'll talk honestly about why this is so hard, the anxiety, the taboo around talking about death, the "I'm healthy" excuse, and walk through what advance care planning actually involves: the difference between advance care planning and advance directives, how to choose a healthcare representative, and the different documents (like a Durable Power of Attorney for Healthcare and a Living Will) that help put your wishes into action.

Location: Zoom Webinar
Format: Presentation and discussion (plan for 75 minutes)

What You'll Learn:

  • The difference between advance care planning & advance directives and why ACP is an ongoing process, not a one-time form

  • Common emotional barriers to advance care planning (anxiety, family dynamics, limited health literacy, and more) and why they're completely normal

  • Six considerations for choosing someone to serve as your healthcare representative, including trust, proximity, and whether they can make hard calls under pressure

  • The different types of advance directives, Durable Power of Attorney for Healthcare, Living Will, and Psychiatric directives, and how each one works

  • How to start these conversations with your medical provider and the people in your life, including real conversation starters you can use

This Workshop Is For You If:

  • You've been putting off advance care planning because it feels overwhelming, scary, or like something for "someday"

  • You're managing a chronic illness and want to feel more in control of your future healthcare decisions

  • You're not sure who to name as your healthcare representative, or what that role actually involves

  • You want practical tools and real conversation starters, not just information to start (or finally finish) your own planning
